Your real name back when you were stationed on the ship. Some times our names change. When you were stationed on the ship, they called you "Richie" and now you call yourself "Richard" or used to be called "Richard" and now you call your self "Bob". Enter the name you used back then to make it easier for your shipmates to recognize you.
Your Nick Name
You know, those cut little Nick names we all ended up with like MUNCHER,  RED, TEX, JUGHEAD. Not a lot of historical value here but fun information to remember.
Name of the Division you were assigned to
What Division (or divisions) were you assigned to? Some of us moved around until we found the division to stay in. You can just put the division you were in for the longest time, or list them all. Examples: A,B,E,R,M,FLAG,H,OE,S1,S2, Marine Detachment.
Your Rate&Rank:
This is your Rate & Rank when you left the ship. Example: BM3, HM2, SN, LT.JG,)
When Did You Come Aboard? Duty Start Month:     Duty Start Year: 
When Did you Leave the ship? Duty End Month:       Duty End Year:  
Shipmates are put onto the crew members web page by the year they came aboard the ship. By listing shipmates by the year they came aboard makes it easier to find the guys you served with. I know it is hard to remember what month you came aboard and what month you left the ship, but try  (or give it a good guess). It helps me put the crew members who served together, listed together on the Crews page.
Your Email Address.
This is how your old shipmates will contact you. Double check your address for correctness! Remember the .com or .net or whatever. If you make an error in the above email information, we will never be able to contact you!  If you have more than one, just give the one you check most often. If you don't have one, you can use someone else's that will get the email to you. Be Careful here! If you type this in wrong, we will never be able to contact you. If you do not hear from me within 10 to 14  days, you should assume that I did not get your request, so please try again.

Is this a Changes Information Request?
If this is a "Change of information" request, please tell us  what information you are changing. If it was your email address, please enter your old email address in the above box so we know what email address to remove from the mailing list. Anything you write in this box WILL NOT show up on the crew members page, this is just notes to the administrator of the crew members pages. Also, you can put anything you want to explain about any of the above answers. .
